1.) SCOPE OF DATA PROVIDED

During the preliminary application phase, the Data Controller only asks for an e-mail address.

The data controller draws attention to the fact that the username and e-mail address do not need to contain personal data. So, for example, it is not necessary that the e-mail address contains your name, you are completely free to decide if you enter a username or e-mail address that contains information referring to your identity.

2.) PURPOSE OF DATA MANAGEMENT, time

The Data Controller processes personal data for the following purposes:

Surname and surname: Entering the surname and surname is necessary because the Data Controller, as a service provider, issues an invoice for the consideration of the service, the mandatory content of which is the full name of the user of the service. In addition, it is also useful in terms of contact if the service provider knows how to call you.

E-mail address: the e-mail address is necessary for communication between the Data Controller and you and for scheduling an appointment.

Billing address: Entering a billing address is necessary because the Data Controller, as a service provider, issues an invoice for the consideration of the service, the mandatory content of which is the address of the user of the service.

Username and Password: The purpose of data management is to ensure the possibility of contact between the Data Controller and you through an online user account, and to simplify contact with you. The username and e-mail address are essential for the identification of the user registering in the database for the purpose of reporting or booking an appointment, and also serves the purpose of maintaining contact between the Data Controller and you. In order to meet data security requirements, entering a password during user identification is mandatory.

The personal data provided by the User is managed by the Data Controller and used exclusively for the purposes indicated above. The Data Controller manages the personal data until you withdraw your consent (delete the registered account). In the case of the family name and first name, as well as the billing address, the duration of the data management is in accordance with the current legal regulations regarding the preservation of the invoice issued for the consideration of the service.

3.) LEGAL BASIS FOR PROCESSING PERSONAL DATA

According to Article 6 (1) point a) of the GDPR, data processing is based on the consent of the data subject, i.e. you, in the case of entering your e-mail address, username and password.

According to Article 6 (1) point c) of the GDPR, data processing is necessary to fulfill the legal obligation of the data controller when providing your family name and surname, as well as your billing address.

4.) DATA TRANSMISSION, KNOWLEDGE OF DATA

The Data Controller and the Data Processor(s) it may use are entitled to access personal data in accordance with the applicable legislation. The Data Controller is not entitled to release any of the Users’ data to third parties, but is entitled to inform registered Users of other, related services in the form of electronic mail.

If it becomes necessary to submit the invoice issued for the service to the tax authority, then the data indicated in the invoice will be forwarded to the state tax authority.

5.) USER RIGHTS

Right of access

The data subject has the right to request information from the Data Controller via the contact details provided in point 1, as to whether his personal data is being processed, and if such data processing is in progress, he is entitled to find out what • the Data Controller – what personal your data; – on what legal basis; – for what purpose of data management; – how long you treat it; furthermore, • to whom, when, on the basis of which legislation, and to whom did the Data Controller grant access to his personal data or to whom did he transmit his personal data; • the source of your personal data; • whether the Data Controller uses automated decision-making and its logic, including profiling. The Data Controller will provide a copy of the personal data that is the subject of data management at the data subject’s request free of charge for the first time, after which it may charge a reasonable fee based on administrative costs. In order to meet the data security requirements and protect the rights of the data subject, the Data Controller is obliged to make sure that the identity of the data subject and the person wishing to exercise their right of access match. For this purpose, information, access to the data, and the issuance of a copy of the data are also subject to the identification of the data subject.
